What's the best way to handle this situation?

Lately I’ve been questioning how effective our Agile process really is. We release features almost every sprint, but afterwards we often realize users don’t actually need what we built, or they use it differently than expected. It feels like we’re very strong on delivery but weak on understanding problems. I keep hearing terms like continuous discovery and continuous delivery, but I’m not fully sure how they’re supposed to work together in real teams. Does anyone have a good explanation of this?

That’s a very common issue, especially for teams that focus heavily on velocity.
