What's the best way to handle this situation?

Lately I’ve been questioning how effective our Agile process really is. We release features almost every sprint, but afterwards we often realize users don’t actually need what we built, or they use it differently than expected. It feels like we’re very strong on delivery but weak on understanding problems. I keep hearing terms like continuous discovery and continuous delivery, but I’m not fully sure how they’re supposed to work together in real teams. Does anyone have a good explanation of this?

That’s a very common issue, especially for teams that focus heavily on velocity. I recently read an article about Continuous Discovery that breaks this down really well — it explains how discovery is not a one-time phase before development, but an ongoing process that should run alongside delivery. The article compares discovery and delivery in modern Agile and DevOps teams and shows why prioritizing only delivery often leads to building the wrong things faster.
